Acknowledgment of Charges:
By accepting this agreement and submitting this form, you approve any charges that may incur related to the resolution of your issue. If you selected “Send Price Estimate Before Starting” you will be provided with a detailed quote for any chargeable services, which must be approved by you before service commencement.
Service Approval:
We will assess the issue submitted and inform you of the resolution. Acceptance of this form gives us approval to move forward with the project as needed.
Payment Obligation:
If you selected “Please Start Immediately” you agree to fulfill the payment obligations required to resolve the issue you requested. If you selected “Send Price Estimate Before Starting” you will be required to approve the quote sent to you and you agree to the payment obligations specified therein.
Cancellation Policy:
If you cancel the requested service after approval but before completion, you may be subject to a cancellation fee.
Warranty and Liability:
All services will be subject to our standard warranty term. Computer Headquarters is not liable for any incidental damages resulting from the servicing of your equipment.
